Little Tip : If your eye care provider doesn't offer you the copy of your eyeglass prescription at the end of the eye examine, remember to ask for it for future convenience.

And there are also some terms that are usually left with blank.

The INT ADD is used for intermediate correction.
Prism
If there is muscle imbalance between your eyes, your Prism can provide the correction and make your eyes work more comfortably. And you might have more than one Prism value.
H-Prism: Horizontal Prism is what put in glasses to compensate for the horizontal misalignment of your eyes.

How to measure your PD?
PD refers to the distance between the two centers of your pupils. We recommend you ask your eye doctor for PD while you are taking an eye test so as to guarantee 100% accuracy. In cases that you have a valid prescription (less than two years old) without PD at hand but need to use it right away, we would love to introduce a simple method to you.
- Find someone for help, friend, partner, anyone is OK.
- Make sure your friend or partner sits at roughly the same height as you do and face you.
- Ask them to hold a millimeter ruler on the bridge of your nose, keep it straight.
- Remind them to adjust the rule "0" until it is exactly in line with the center of your left pupil.
- Ask them to read the measurement between the left pupil and right pupil. (Remember to look ahead while they are reading the measurement).
- Repeat it 3 to 4 times for accuracy.
Little Tip: In order to make the distance between the optical centers of your glasses lenses the same as the distance between the centers of your eyes, Pupillary Distance is the key. So if you still don't have the correct PD, remember to ask for it from your eye doctor next time you have your eyes examined. PD won't change if you are an adult, so one correct PD is enough for life-long use.
